We are currently seeking a dedicated and compassionate Part-Time Licensed Master Social Worker (LMSW) to join our growing team in the DFW Metro area.

Position Summary

The Hospice Social Worker provides essential psychosocial support to hospice patients and their families. This role focuses on counseling, care coordination, resource connection, and emotional support during end-of-life care.

Hours are flexible and based on the needs of the assigned patient caseload.

Key Responsibilities

Conduct psychosocial assessments for hospice patients and families

Provide emotional support, counseling, and crisis intervention as needed

Assist patients and caregivers with advance care planning and end-of-life resources

Collaborate with the interdisciplinary hospice team to develop and implement the plan of care

Provide education and support related to grief, loss, and coping strategies

Connect families with community resources, financial assistance, and support services

Participate in care conferences and documentation per hospice regulations

Maintain accurate, timely clinical documentation in compliance with Medicare and hospice standards

Qualifications

Active LMSW license required (Texas)

Master’s degree in Social Work (MSW)

Previous hospice or healthcare experience preferred

Strong interpersonal, communication, and organizational skills

Compassionate approach to supporting patients and families during end-of-life care

Ability to work independently and manage a flexible caseload schedule

Reliable transportation for travel within the DFW Metro area
