    Part Time LPN – Assistant to the RCD (Care and Administration)

    $30-$35/hour based on experience

    We are seeking a skilled and experienced LPN/LVN to join our team in a part time care and administrative capacity at Aviva Senior Living in Toledo, Ohio.     Shift Details

    Approximately 20 to 25 hours per week, Monday through Friday, roughly 4 hours per day. The schedule is flexible and negotiable. An optional on call weekend rotation may be available for those interested, and PRN on call shifts can also be picked up as desired.


    Primary Responsibilities

    Administrative Support

    • Provide direct support to the Resident Care Director (RCD) in day to day administrative functions
    • Assist with Medicaid documentation, compliance, and coordination
    • Maintain accurate and up to date medical records and care plan documentation
    • Assist with scheduling, reporting, and other administrative tasks as needed

    Clinical Leadership and Mentorship

    • Serve as a resource and mentor to other LPNs on the team
    • Support clinical oversight and help ensure consistent, high quality care standards
    • Communicate effectively with residents, families, and the broader care team
    • Collaborate with healthcare professionals to support comprehensive resident care

    As Needed Clinical Coverage

    • Administer medications and treatments as prescribed
    • Monitor and record vital signs including blood pressure, temperature, and pulse
    • Assist with resident assessments, care plans, and diagnostic procedures
    • Provide direct patient care as needed during on call or PRN shifts

    Qualifications, Skills, and Abilities

    • Active LPN license valid in Ohio
    • Minimum of 2 years of LPN experience; senior living or long term care experience preferred
    • Familiarity with Medicaid processes and documentation is a strong plus
    • Strong communication and interpersonal skills
    • Ability to work both independently and collaboratively within a multidisciplinary team
    • Excellent organizational skills and attention to detail
    • Ability to prioritize and manage multiple responsibilities
    • Basic computer proficiency
    • A genuine commitment to treating residents, families, and team members with respect and compassion

    Compensation and Benefits

    Pay advance (daily pay) is available. A full benefits package is available upon advancement to full time status. Final compensation will be dependent on experience, qualifications, and other relevant factors.
