Responsibilities: • Build and maintain relationships with physicians, hospitals, SNFs, and referral sources • Generate patient referrals and grow agency census • Conduct regular in-person and virtual outreach within the assigned territory • Educate partners on services, specialties, and admission criteria • Coordinate with intake and clinical teams to convert referrals into admissions • Ensure timely collection of required documentation (orders, insurance, F2F) • Track referrals, marketing activity, and performance metrics • Analyze market trends and identify new business opportunities • Represent the agency at meetings, events, and community outreach • Distribute and present marketing materials professionally • Support payer relationships and contracting efforts • Ensure compliance with Medicare, HIPAA, and marketing regulations • Manage territory with structured weekly visit plans • Collaborate with clinical staff to align services with referral needs • Meet or exceed monthly referral and admission targets Qualifications: • Minimum 3-5 years of experience in home health or healthcare marketing • Proven ability to build relationships with physicians, hospitals, case managers, and referral sources • Strong knowledge of home health services, Medicare guidelines, and insurance basics • Excellent communication, presentation, and interpersonal skills • Ability to work independently and manage a designated territory effectively • Strong organizational skills with the ability to track referrals and follow up consistently • Proficiency with CRM systems, Microsoft Office, and/or Google Workspace • Valid driver’s license, reliable transportation, and willingness to travel locally Preferred / Bonus Qualifications • Existing relationships with referral sources in Contra Costa, Alameda, or San Joaquin counties Compensation: $25 - $30 hourly
