    Part Time Community Homes QDDP

    Looking for a position that offers growth working with clients with diverse disabilities in a community setting? This position will work directly with a seasoned, full-time QDDP who will provide training and mentorship. This part-time position will have opportunity to be exposed to all facets to ensure all our clients thrive and become the best version of themselves.

    • Specific Roles and Responsibilities:

    o Creating strong client-based teams with face-to-face client team meetings

    o Ensuring all documentation remains up to date and filed onto the LBSA server and Therap. Also ensure signatures are collected for all documents that require signatures.

    o Monthly data collection and analysis to ensure programs are running smoothly and staff are documenting things correctly.

    o Complete Pre Meeting checklist 2 weeks in advance for meetings (Includes mailing out meeting documents and reminder and prepping documents to mail out.)

    o Attend client meetings as they are scheduled.

    • Accountabilities:

    o Representing the Association in a positive and professional manner and adhering to the policies and procedures of Laura Baker Services Association.

    o Communicating in an open and professional manner within the households, with clients and their families, LBSA staff, support staff, other professionals, and members of support teams.

    o Effectively able to communicate information, provide support, and serve as resource to clients and staff.

    o Genuine interest in evaluating compliance with licensing and regulatory standards.

    o Developing meaningful recommendations for programming, considering input from clients and staff and revising goals and behavior programs as needed, using a variety of teaching techniques, and incorporating LBSA philosophy.

    o Monitoring the accomplishment of outcomes for each client as specified in the IPP (Individual Program Plan), and taking action necessary to facilitate the accomplishment of outcomes

    o Creating/maintaining CSSP (Coordinated Services and Supports Plan) to ensure compliance with provider responsibilities.

    o Communicating to the Internal Review Committee regarding the restrictions of rights.

    o Analyzing incident reports (GERs) in a timely fashion, making note of patterns in behavior, and recommending changes in individual programming, and/or other service delivery methods when needed.

    o Analyzing risk and writing Abuse Prevention Plans and service assessments in cooperation with Household Director.

    o Reviewing program data (daily logs, goal data, quarterly reports, and annual reports) in a timely manner, and making explicit and knowledgeable recommendations for programming changes, when needed.

    o Evaluating and identifying general and specific staff training needs

    o Mentoring and instructing staff on a variety of topics, including but not limited to IPP implementation and attending staff meetings as needed.

    o Investigating and receiving maltreatment reports
