What You’ll Do

Conduct in-home client assessments

Develop individualized plans of care

Provide nursing interventions focused on realistic client outcomes

Update care plans based on client progress

Maintain accurate clinical documentation

Support caregiver onboarding, skills assessments, drug testing, and training

Perform additional clinical tasks as needed

What You Bring

Current RN license

Graduate of an accredited nursing program

CPR certification

Reliable transportation

Strong communication and documentation skills

Geriatric or home care experience preferred
