Part-Time Registered Nurse (RN) - San Diego

Location: Various patient homes throughout San Diego County (including El Cajon, La Mesa, Chula Vista, etc.).Schedule: Part-Time; consistent assignment potential based on patient needs and RN availability. Reports To: Clinical Supervisor

Core Responsibilities:

Perform comprehensive skilled nursing visits according to physician orders and the established plan of care.

Conduct initial patient assessments, ongoing monitoring, and necessary interventions.

Manage and administer medications and treatments safely and accurately.

Provide thorough patient and family education regarding diagnoses, treatment plans, and self-care management.

Maintain accurate and timely documentation in the electronic health record (EHR) system.

Collaborate with the home health team (PT, OT, HHA, etc.) to ensure coordinated care.

Essential Qualifications:

Active, unrestricted Registered Nurse (RN) license in the State of California.

Minimum of one (1) year of recent clinical experience (Home Health experience is a strong plus!).

Current BLS/CPR Certification.

Must have reliable, insured transportation to travel to patient homes within San Diego County.

Strong foundation in clinical assessment and professional communication.
