Family Medicine Physician – Primary Care | Brockport, NY

Provide comprehensive primary care: diagnosis, treatment, coordination, preventive care, and health maintenance.
Lead the care team in clinical decision-making and provide supervision/collaboration for nurse practitioners and physician assistants.
Conduct thorough patient histories and physical examinations.
Prescribe medications per state and professional practice guidelines.
Perform minor procedures and generate specialty referrals as needed.
Evaluate lab results, diagnostic reports, and follow up with patients accordingly.
Maintain accurate and timely chart documentation using EHR.
Adhere to clinical productivity and scheduling guidelines.
Participate in team meetings, peer reviews, and quality improvement initiatives.
Support outreach, health education, and community-based care programs.
Stay compliant with all federal, state, and local regulations.
MD or DO from an accredited medical school.
Active New York State medical license and current DEA certification.
Board Certified or Board Eligible in Family Medicine (must become certified within 18 months if eligible).
Current CPR certification.
Strong communication, collaboration, and time management skills.
Proficiency in electronic health records and basic computer systems.
Bilingual or multilingual providers encouraged to apply.
Experience in community-based or rural primary care settings.
