    Family Medicine Physician – Primary Care | Brockport, NY

We're seeking a dedicated Family Medicine Physician to provide full-scope primary care services in Brockport, NY.     Responsibilities:

    • Provide comprehensive primary care: diagnosis, treatment, coordination, preventive care, and health maintenance.

    • Lead the care team in clinical decision-making and provide supervision/collaboration for nurse practitioners and physician assistants.

    • Conduct thorough patient histories and physical examinations.

    • Prescribe medications per state and professional practice guidelines.

    • Perform minor procedures and generate specialty referrals as needed.

    • Evaluate lab results, diagnostic reports, and follow up with patients accordingly.

    • Maintain accurate and timely chart documentation using EHR.

    • Adhere to clinical productivity and scheduling guidelines.

    • Participate in team meetings, peer reviews, and quality improvement initiatives.

    • Support outreach, health education, and community-based care programs.

    • Stay compliant with all federal, state, and local regulations.

    Requirements:

    • MD or DO from an accredited medical school.

    • Active New York State medical license and current DEA certification.

    • Board Certified or Board Eligible in Family Medicine (must become certified within 18 months if eligible).

    • Current CPR certification.

    • Strong communication, collaboration, and time management skills.

    • Proficiency in electronic health records and basic computer systems.

    Preferred:

    • Bilingual or multilingual providers encouraged to apply.

    • Experience in community-based or rural primary care settings.

    Compensation & Benefits:
