  • Responsibilities

    Partners In Care is seeking Support Registered Nurses to deliver comprehensive care and education to our hospice and home health patients. Collaborating with an interdisciplinary team of social workers, RN case managers, chaplains, volunteers and more to enhance the lives of our patients and their families.

    Compensation: $40.00-$58.67 (DOE). Differentials $5/hour nights & $2/hour weekend.

    Sign-on Bonus: $2,000, paid after 90 days of employment.

    Location: Positions in Bend and Redmond/Madras (North).

    Travel: Local/regional travel by personal vehicle during the working day, centralized from the Bend office and extending throughout Central Oregon. Mileage reimbursement is provided.

    What You Get To Do:

    Complete a comprehensive and accurate assessment of the patient and family to determine health needs.

    Coordinate patient care with the patient’s case manager. Provide verbal or email updates as necessary.

    Provide a complete physical assessment and history of previous illness (es) and use the health assessment data to determine the appropriate diagnosis and need for other disciplines and resources.

    Provide nursing procedures and treatments and end-of-life comfort procedures per care plan. Administer medications and treatments as prescribed by the physician.

    Provide health care instructions and education to the patient and or family as appropriate per the assessment plan and patients’ caregivers’ ability to learn. Adapt teaching styles to meet patient’s needs and family’s needs.

    Communicates with the physician regarding the patient’s needs, reports, and changes in the patient’s condition; obtains/receives physician’s orders as required.

    Communicate with all healthcare team members to promote coordinated, efficient care and document coordination of care with all involved with patient care.

    Participate in on-call duties as defined by the on-call policy.

    Required Skills & Qualifications:

    Valid Oregon RN License.

    Ability to pass a background test, drug screen, and TB test.

    Graduate of an accredited nursing school.

    One to two years of recent medical care experience in an institutional setting is preferred. Ideal candidates will possess 1-2 years of home health or hospice experience.

    Current automobile insurance required. A valid Oregon driver’s license and reliable transportation.

    Valid CPR Certification.

    Strong communication and interpersonal skills.

    Ability to work independently and collaboratively within an interdisciplinary team.
