  • Responsibilities

    Partners In Care is seeking an RN Case Manager to deliver comprehensive care and education to our hospice patients. Collaborating with an interdisciplinary team of social workers, Support RNs, chaplains, volunteers and more to enhance the lives of our patients and their families.

    Compensation: $40.00-$58.67 (DOE). Differentials $5/hour nights & $2/hour weekend.

    Sign-on Bonus: $2,000 paid after 90 days of employment. Location: La Pine, Oregon.

    Schedule(s) Available: Full-Time: Monday-Friday 8:00 am-5:00 pm.

    Travel: Local/regional travel by personal vehicle during the working day, centralized from the Bend office and extending throughout Central Oregon. Mileage reimbursement is provided.

    What You Get To Do:

    Complete an initial comprehensive and accurate assessment of the patient and family to determine health needs.

    Develop and update individualized care plans, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions.

    Coordinate appropriate preventive and rehabilitative nursing procedures, treatments, medication, equipment, and end-of-life comfort procedures as prescribed by the physician.

    Provide health care instructions and education to the patient and or family as appropriate per the assessment plan and patients’ caregivers’ ability to learn.

    Identify discharge-planning needs as part of the initial care plan development and implement them prior to the patient's discharge.

    Develop, monitor, and update the Home Care Aid care plan as required.

    Communicate with all healthcare team members to promote coordinated, efficient care and document coordination of care with all involved with patient care.

    Participate in weeknight and weekend on-call duties as defined by the on-call policy.

    Required Skills & Qualifications:

    Valid Oregon RN License.

    Ability to pass a background test, drug screen, and TB test.

    Graduate of an accredited nursing school.

    One to two years of recent medical care experience in an institutional setting is preferred. Ideal candidates will possess 1-2 years of home health or hospice experience.

    Current automobile insurance is required. A valid Oregon driver’s license and reliable transportation.

    Valid CPR Certification.

    Strong communication and interpersonal skills.

    Ability to work independently and collaboratively within an interdisciplinary team.
