EDUCATION: MASTER’S IN PUBLIC HEALTH, SOCIAL WORK OR OTHER RELATED FIELD AND AT LEAST 5 YEARS OF RELEVANT EXPERIENCE IN THE FIELD OF EVIDENCE-BASED PREVENTION PROGRAMS AND/OR PROGRAM EVALUATION.

Job Summary:

·      Directs the Administration for Community Living Chronic Disease Self-Management Education Programs and other evidence-based programs.

·      Directs Partners in Care’s Title IIID contract from the Los Angeles Department on Aging to spread evidence-based programs within the City of Los Angeles.

·      Provides technical assistance on evidence-based program implementation to partner organizations and affiliates statewide.

·      Engages in broad strategic planning to generate innovative and sustainable partners and strategies and identify long-term funding streams to embed evidence-based programs in organizations for seniors and adults 18+ with disabilities.

·      Experience in Public speaking for presentations.

·      Persuasive communication skills for presentations.

·      Experience in creating and monitoring budgets and business plans.

·      Ability to create positive working relationships with a wide variety of people and organizations.

This position will be responsible for Program Direction for: 1) the implementation of our City of Los Angeles IIID grant, 2) Administration for Community Living grant related to self-management, 3) development of new business partnerships with the Vice President,

4. and will work closely with staff on community-wide coalitions to spread our evidence- based work.

Responsibilities:

1.      Engages in broad strategic planning with top management of health plans and systems, foundations, education, non profits and aging organizations to generate innovative and sustainable partners and strategies and identify long-term funding streams to embed evidence-based programs within the state, county and City of Los Angeles.

2.      Participates in identifying funding opportunities, responds to Requests for Proposals (RFP), and prepares competitive grant proposals that meet organizational and project goals.

3.      Designs achievable project workplans, protocols and procedures for network of partners and monitors activities to ensure grant objectives and commitments are fulfilled.

4.      Engages in group problem solving with multiple partners to generate innovative and creative approaches and solutions to challenges and uses feedback to modify strategies to mobilize partners to fulfill vision of initiative.

5.      Aligns grant deliverables and activities with strategic organizational goals of Partners in Care.

6.      Responds to inquiries from prospective organizations to explore participation and readiness to engage in initiative.

7.      Directs and supervises project team activities, delegates and monitors meaningful work assignments, promotes ability to work independently and builds positive team culture.

8.      Develops and conducts persuasive presentations at local, state and national conferences and meetings to top management, funders, public groups and prospective partners to communicate initiative goals and objectives and to recruit new partners.

9.      Monitors evaluation procedures, data compliance and quality assurance to ensure progress is accurately reported to funders.

10.  Monitors multiple budgets and fulfills reporting requirements and grant objectives in compliance with funding streams.

11.  Manages coalition development and evidence based program network development. Specifically provides support to Vice President related to coalitions with LADOA, LADPH, LA County AAAs, Kaiser, UCLA, USC and other EBP providers.

12.  Tracks trends, analyzes emerging issues, and initiates dialogue to determine the appropriate response to community needs, both internally and externally.

13.  Identify, track, and document case studies of best practices among grantees and other partners. Responsibilities may include developing and facilitating focus groups, developing surveys and conducting program evaluation.

Key characteristics, skills and competencies include:

·         Bilingual in Spanish or other language (Mandarin, Cantonese, Korean, etc.) strongly preferred

·         Flexibility and creativity

·         Three or more years managing a professional staff

·         Proof of being an effective professional team member

·         Knowledge of Evidence-Based Programs including; CDSMP/DSMP/PSMP/Tomando Control, Arthritis Exercise, A Matter of Balance

·         Business development experience

·         Ability to manage the progress of department activities to ensure that goals and objectives are met

·         High professional standards and expectations

·         Good judgment and an ability to exercise discretion

Minimum Qualifications:

·         Master’s in Public Health, Social Work or other similar degree and at least 5 years of relevant progressive experience in project management in the field of healthy aging, evidence-based programs, and/or program evaluation; or an equivalent combination of training and experience related to the duties of the position.

·         Excellent oral and written communication skills.

·         Highly motivated and independent performer; enthusiastic about working in a team environment to improve the lives of underserved individuals.

·         Excellent interpersonal skills; ability to work well with all styles, internally and externally is a must. Must be a team player, have lots of patience, humility and flexibility.

·         Excellent computer skills in Microsoft Office to include Word, Excel, PowerPoint and database experience.

·         Possession of a valid California driver's license and proof of auto insurance.
