We have over 30 outpatient behavioral health locations nestled throughout our communities located in Kern County, Los Angeles County, Orange County, and San Diego County. Opportunity Knocks is a volunteer mental health program providing outpatient services for individuals between the ages of 18-59, with severe and persistent mental illness, recent history of incarceration due to mental illness, and are currently homeless or at risk of homelessness. Our services include intensive case management, psychotherapy, skill-development groups, medication management, vocational linkage, temporary housing support, benefits processing, and outreach and engagement.

    SummaryThe Personal Service Coordinator-- Outreach and Engagement engages in community outreach activities including canvassing local public areas to provide assessment, crisis intervention, short term case management, educational and life skills training, individual, group, and family counseling, as well as clinical documentation of services to potential and actual participants of the identified program. Participation as a member of the multidisciplinary team that plans and implements participant care and discharge planning when facilitating community reintegration. Maintains and meets expectations on time for all competencies, certifications and education requirements as outlined by Pathways, Centers for Medicare and Medicaid Services (CMS), and all other regulatory agencies.Essential Duties/Responsibilities▪ Demonstrate and possess the ability to assess data reflective of the participant’s status and interpret relative to the participant’s age-specific needs, and to provide the services needed.▪ Provide intensive case management services and mental health rehabilitation counseling, as needed.▪ Maintains daily documentation of work activities▪ Document all services provided within 24-48 hours and ensure timely corrections, if needed.▪ Document all services as required to meet agency expectations and Medi-Cal and Medicare standards.▪ Be self-directed and able to work well in a team.▪ Develop and maintain professional working relationships with agencies and service providers.▪ Meet monthly productivity requirement of 100 direct service hours (DSH) or otherwise required by the program.▪ If caseload assignment required, maintain 100% charting compliance.▪ Consults with Program Director and/or Supervisor regularly on recovery care plans for participants.▪ Attends and participates in weekly staff meetings and all other meetings as required.▪ Attends and participates in treatment team meetings as scheduled and brings any clinical or medical concerns regarding specific participants to the attention of the treatment team.▪ May be required to handle on-call and work a flexible schedule to meet program and participant needs.▪ Conduct field-based outreach of up to 70% or as required by the program.▪ Other duties as assigned or necessary to support the program and/or the company.Recovery Requirements• Provide the best customer care possible.• Identify and build upon the strengths of participants, coworkers, and the communities we serve.• Support participants’ steps towards Recovery and Wellness.• Create an organizational culture that respects and celebrates the diversity of our participants.• Value learning as an ongoing process that enables us to better service our consumers and establishes our leadership in the industry.• Research and utilize our industry’s best practices and analyze our own services to ensure the best possible outcomes.Education and/or ExperienceBachelor’s degree from an accredited university, and a minimum of 1 year experience in social/human services. Prior behavioral health experience preferred.Certificates, Licenses, RegistrationsProof of: valid CA driver’s license, valid auto insurance coverage as well as proof of education are required. Prior behavioral health experience preferred.Knowledge, Skills and Experience▪ Working knowledge of DSM V, SBIRT, ASAM, and psychiatric disorders preferred.▪ Basic PC/computer skills including knowledge of Microsoft Word, Excel and PowerPoint and electronic charting▪ Demonstrates the knowledge and skills necessary to provide care/service appropriate to the age of the patients/individuals served (Ages Served: Transitional Age Youth (16-25)).▪ Knowledge of SAMHSA Recovery Philosophy and rehabilitation model.▪ COD (co-occurring disorders, dually diagnosed) experience a plus.▪ Excellent organizational skills, interpersonal skills, computer skills and communication skills are a must.▪ Ability to effectively prioritize and manage time.▪ Preferred language and cultural background that meets the County’s threshold requirements.Physical Requirements▪ Office Environment. Requires extensive sitting with periodic standing and walking.▪ While performing the duties of this job, the employee is frequently required to stand; walk and sit.▪ May be required to lift up to 20 pounds.▪ Requires significant use of computer, tablet, phone and/or general office equipment.▪ Needs adequate visual acuity, ability to grasp and handle objects.▪ Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone.▪ Requires off-site travel of up to 70% of position responsibilities; off-site travel requirements may vary per program.
