    Position Overview

    The Intake Clinician evaluates and collects client information for the assessment of therapeutic services, and determines initial service needs.

    General Description

    The Intake Clinician is the initial clinical point of contact for clients and their families. S/he will obtain demographic, general history and current presenting issues in order to make a clinical diagnostic impression and make recommendations for therapeutic services. They may carry an assigned caseload and will provide services as part of the assessment and transition process. Such services may be delivered in-office, in-home and/or in community based sites.

    The Intake Clinician is responsible for performing assessments, making treatment recommendations and knowledge of agency services and community resources. Services provided to clients and families, include, but are not limited to: best practices in therapy; developing treatment goals and objectives, mutually developed by the family and clinical team; and case management duties such as: liaison and advocacy roles; coordinate staffing with family and other team members; system navigation and community referrals as needed. The Intake Clinician may coordinate with the referral agent, and gather information from community resources which may include schools, PCP, PCJCC, or other necessary resources.

    The Intake Clinician will complete the intake/assessment through the Electronic Medical Record program.

    Education, Certifications and License Requirements

    Bachelor’s Degree in Social Services, Psychology or related field or a non-related Bachelor’s degree and 1 year work in the mental health field; or a Master’s Degree in Counseling, Therapy, Psychology or related field by an accredited educational institution with relevant experience.

    Must have current fingerprint clearance (and maintain during employment) and be able to pass the background check. Proof of: current TB test, CPR/FA and a valid Arizona driver’s license as well as proof of education are required. Prior behavioral health experience preferred. Bilingual-Spanish/English a plus.
