Pathways is seeking an innovative and purpose driven individual to join our team as an Independent Life Skills Coach for our STAY program.

STAY Process provides personalized and recovery-oriented mental health services to Transitional Aged Youth (16-25 years old) with severe and persistent mental illness, who are homeless or at risk of homelessness. Working with a multidisciplinary team, transitional aged youth receive individualized goal setting, intensive case management, medication management, temporary assistance with housing, educational services, employment services, substance abuse services, skill-development groups and community based activities.

Life Skills Coaches are fully integrated team members who provide highly individualized services in the community and promote client self-determination and decision-making. Life Skills Coaches also provide essential expertise and consultation to the entire team to promote a culture in which each client's point of view and preferences are recognized, understood, respected, and integrated into treatment, rehabilitation, and community self-help activities.

SALARY: $19.00 - $21.00 an hourESSENTIAL JOB FUNCTIONS AND REQUIREMENTS• Carry out duties as a member of the team, under the direction of the team leader and the team psychiatrist. Specific job responsibilities will vary depending on education and experience • Be a member of an individual treatment team for a number of clients• Provide work-related supportive services, such as assistance securing necessary clothing and grooming supplies, wake-up calls, transportation• Perform mentoring, problem solving. Offer encouragement on and off the job site• Assist and support clients to carry out personal hygiene and grooming tasks• Provide practical help and mentoring, advocacy, coordination, side-by-side individualized support, problem solving and direct assistance to help clients obtain the necessities of daily living.• Other duties as assigned or necessary to support the program and/or the company.• Provide the best customer care possible.• Identify and build upon the strengths of consumers, coworkers, and the communities we serve.• Support consumers’ steps towards Recovery and Wellness.• Create an organizational culture that respects and celebrates the diversity of our consumers.• Value learning as an ongoing process that enables us to better service our consumers and establishes our leadership in the industry.• Research and utilize our industry’s best practices and analyze our own services to ensure the best possible outcomes.QUALIFICATIONS• High school diploma or GED and 1 year of minimum experience required • Valid Driver's License and proof of automobile insurance
