Pathways is looking for a Part-Time (16 hours) Nurse Practitioner to join our Oasis program, under the direction of a medical doctor performs comprehensive psychiatric evaluations, assessments, diagnosis, medication management, evaluation and on-going treatment of clients with mental disorders.
Oasis program is a recover-oriented older adult behavioral health program providing intensive field-and clinic-based wraparound outpatient services for individuals ages 60 and over diagnosed with a SPMI resulting in significant functional challenges in daily living and that are currently homeless or at risk of homelessness. Services include outreach and engagement, personalized goal setting, multi-disciplinary team care, intensive case management, co-occurring substance abuse and mental health treatment, psychotherapy, family support, skills-developing groups, medication management, benefits and vocational assistance, crisis interventions, and short-term temporary housing support. Oasis primarily serves individuals with Med-Cal, and Medicare, or who are unfunded.
Essential Duties/Responsibilities
Conducts initial client assessment to include obtaining psychiatric and medical history, performing psychiatric assessment and overall health assessment for any presenting problem
Orders necessary laboratory studies, monitors and follows-up on abnormal results
Provides prescriptions for psychiatric medications under the advice and direction of a medical doctor
Assesses and evaluates responses to medications and monitors for potential side effects or adverse reactions
Consults with other members of the treatment team on clients’ treatment plans
Documents using Anasazi Electronic Health Record, a complete assessment and plan including all medication changes on each client and all other mandatory documents as designated by Program Director
Obtains medication consents when appropriate
Participates in staff in-service and case presentation meetings
Consults with medical doctor regularly on treatment plans for clients
Perform duties as designated by ACT model treatment modality
Perform Home visits and off-sites visits as required by the needs of the families and participants
Provide support and perform other duties as assigned or necessary
May provide direct supervision to any LPT or LVN’s on staff
Works under minimal supervision by Program Director and Staff Psychiatrist

Education/Licensing Requirements
•Graduate of an accredited college or university with a masters in Nursing•Current Nurse Practitioner license from the Board of Registered Nursing in the practicing state
