    Pathways is seeking an innovative and purpose driven individual to join our team as a Intake Coordinator at our Catalyst in San Diego.Catalyst

    is an Assertive Community Treatment (ACT) program. Catalyst provides full service partnership services to Transitional Age Youth (TAY) ages 16 to 25 with a serious mental illness, and who are homeless or at risk of becoming homeless in San Diego County. Catalyst’s ACT team provides comprehensive, individualized services in an integrated and continuous manner. Services are collaborative and recovery-oriented, and treatment includes: psychiatric assessment, medication management, individual therapy, substance abuse treatment, co-occurring disorders treatment, supportive housing, resource linkage, advocacy, family psycho-education and therapy, supportive employment and education development, legal assistance, community integration services, peer support and group counseling.

    ABOUT YOUR ROLE

    Intake Coordinator is responsible for coordinating the needs of each client and family on their caseload.

    ESSENTIAL DUTIES/RESPONSIBILITIES

    The intake coordinator position provides outreach and assessment to prospective clients.

    Works collaboratively with referral source for coordination and continuity of care.

    Responsible for conducting initial visit to determine if the member meets program criteria.

    The Intake Coordinator enrolls the member into the program, completes all appropriate paperwork and obtains signatures following the enrollment process before assigning the member to their Care Manager or Clinician.

    Completes comprehensive evaluation/assessment of each client’s functioning, including problems and needs, and obtains consumers consent to participate.

    In conjunction with consumer, the Intake Coordinator develops a detailed care plan.

    Once the assessment has been completed staff will confer with the clinical manager to ensure that all admission requirements have been met prior to enrollment.

    Maintains daily record of work activities; completing daily service progress notes and billing forms per Company and government standards and in compliance with Short Doyle/Medi-Cal requirements.

    Develop and maintain professional working relationships with agencies and service providers.

    Monthly Productivity requirement is 45 DSH/direct service hours.

    Other duties as assigned or necessary to support the program, office and/or the company.

    DO THE FOLLOWING APPLY TO YOU?

    EDUCATION, CERTIFICATIONS AND LICENSE REQUIREMENTS

    Education and/or Experience

    Master’s degree from accredited school in Social Work, Psychology, Counseling, or related field.

    Prior behavioral health experience preferred

    Certificates, Licenses, Registrations

    Must be registered as an ACSW, AMFT or APCC or Licensed LCSW/ LPCC / LMFT

    Proof of: valid California driver’s license and auto insurance coverage, as well as proof of education are required.

    KNOWLEDGE, SKILLS, AND EXPERIENCE

    Will be required to learn and have knowledge of "recovery model".

    Ability to communicate effectively, both verbally and in writing

    Intermediate PC skills

    Excellent documentation and interpersonal skills

    Ability to effectively prioritize and manage time

    Ability to perform basic word processing and utilize Electronic Medical Records (EHR) system for the completion of tasks or has the ability to learn these skills.
