    Pathways of Idaho is looking for a Licensed Clinician for the new Pathways Community Crisis Center of Southwest Idaho! Southwest Idaho's only Behavioral Health Crisis Center is looking for LSWs, LMSWs, LCSWs, LPCs, LCPCs, LMFTs to perform behavioral health assessments and de-escalation on participants who present to the center as experiencing a behavioral health crisis.     Qualifications:

    Evidence of current qualified Idaho licensure is required

    Individuals with licensure as a licensed Masters social worker are subject to provisions of supervision as identified by Optum. Practice is not allowed independent of supervision

    Must be able to pass background check

    Ability to perform basic word processing tasks and utilize proprietary software for the completion of tasks, or has the ability to learn these skills

    Responsibilities:

    Maintaining functions within the scope of practice as established by the contract developed through DHW regarding the PCCCSI

    Facilitates completion of all initial behavioral health assessments when clients experiencing a behavioral health crisis present at the crisis center, within 1 hour of client’s arrival.

    Identifies client’s immediate needs to facilitate discharge from the crisis center.

    De-escalates clients experiencing an acute behavioral health crisis.

    Ensures delivery of services as identified in the crisis center contract, managed by the Department of Health and Welfare. Typical services may include but are not limited to: crisis and risk factor assessment, providing emergency/crisis psychotherapy services including de-escalation for clients experiencing acute mental health crisis, providing individual/group/family psychotherapy, coordinating/collaborating with identified treatment team members, creating discharge/aftercare plans to ensure ongoing stabilization for clients leaving the crisis center, and reporting issues of legal, ethical or safety to the proper channels.

    Documents all provided psychotherapy services in client’s record as established by the contract managed by the Department of Health and Welfare. Typical tasks include, but may not be limited to: accurate documentation of participant assessments, Crisis Evaluation in WITS, Risk Assessments (i.e. the one in WITS and any additional ones identified), Mental Status Exam (on paper and placed in client’s chart), and Treatment Plan in WITS. As well as Encounter / Miscellaneous (DAP progress notes in WITS) and Periodic reviews (Updates in WITS). Termination summaries/ Discharge Plans (under DAP notes in Progress notes in WITS)

    Collaborates routinely with State Clinical Director, supervising physical/Medical Director, Crisis Center Program Manager, clients and other key personnel as identified as essential to client care.

    Demonstrates competence/knowledge with regards and implementation of both effective psychological theories and clinical practice.

    If applicable completes required continued education annually in the field of licensure as required by the State of Idaho Licensing Board. Maintains current professional licensure within field of practice to the standards/ requirements as set by the Bureau of occupational Licenses.

    Maintains confidentiality of all client documents, files and communication in written, electronic and/or verbal form

    Engages in team activities for the client and in order to meet client needs such as, but not limited to, completing team staffings after every client’s admission and at the start of every therapist’s shift, providing for client with regard to creating a welcoming atmosphere, and maintaining a clean and positive environment (i.e. picking up after a client whom nursing staff puts in the shower, giving meals and cleaning up leftovers after the client eats, completing laundry and other chores).

    Leads the team in staffings after each client’s assessment and at the start of every therapist’s shift. Leads the team in making decisions for client’s care while in the facility and regarding discharge plans by engaging each discipline in the decision-making process.

    Completes wellness and safety plan prior to discharge if client is at high risk for suicide or homicide (instead of CM and PS completing).

    Monitors client safety such as checking on clients every minute when client is in the bathrooms, observing clients in the day and sleeping areas, talking with clients in between assessment and discharge, reassessing clients that are suicidal or homicidal prior to discharge.

    Participate in any in-service training for team members

    When necessary, provides in-service training to staff regarding clinical and psychiatric issues.

    Writes client information on triage board and identifies acuity level of client for treatment team.

    Completes intake form by identifying if it is a full or partial admission and by putting a Z-code on it.

    Notifies staff designated per shift/team responsible for inventory of any medical supply needs.

    Completes other duties as assigned or necessary to support the program/ company.
