    About your role: Our Care Coordinator provides Targeted Case Management (TCM) and Behavioral Health Home (BHH) services to children and adults with a behavioral or mental health diagnosis. You will meet with your clients and families in their homes, to assess their strengths and emotional, behavioral, and physical needs, linking them to appropriate community resources and creating a targeted treatment plan. You will meet with the client and families regularly to engage them in their progress and success, and collaborate with educational, vocational, medical, and behavioral health providers to provide wrap around services.

    What we are looking for at least one of the following:

    Bachelor’s degree in any field

    Experience or degree in Social, Behavioral, or Health Services preferred

    MHRT-C preferred
