    Summary

    A Community Support Associate Professional functions within the scope of practice as established by Washington State Department of Health and Department of Social and Health Services to provide individualized transition planning services to clients in need of transitioning from an institutional setting to a less restrictive level of care setting. Qualifying individuals include adults on Roads to Community Living or Washington Roads. Part time 15 hours per week.

    Essential Duties/Responsibilities

    Conduct a pre-transition consultation and determine viability for clients interested in transitioning from an institutional setting and transition/support needs. Completion of Individualized Transition Plan.

    Where relevant, act as a liaison with and among the client, nursing or institutional facility staff, case managers, housing providers, medical personnel, legal representatives, formal caregivers, family members, informal supports and any other involved party.

    Provide assistance and support to ensure the client’s successful transition from the institutional setting. Provide the client with transition services in a manner consistent with protecting and promoting the client’s health and welfare, and appropriate to the client’s physical and psychological needs.

    Assist with locating and arranging transportation resources to effectively connect with client and community resources.

    Assist with filling out forms and obtaining needed documentation to aid in the client’s successful transition from the institutional setting.

    Locate and arrange appropriate, accessible housing. Work with housing authorities and other community resource providers regarding housing availability, subsidies and voucher capability. Arrange the client’s physical move from the institutional setting. Establish change of address with all necessary parties including the US Postal Service, case manager and when relevant, RCL Housing Specialist.

    Assist with ensuring medical resources are available and met. This could include locating and making arrangements with pharmacies, primary care physicians and other necessary medical providers.

    Connect client with less formal or atypical daily activities, such as those that might be found through YMCA-type organizations, senior or community centers, etc.

    Preform transition planning activities within the scope of practice of the contractors’ license and in compliance with professional rules, as defined by law or regulation, as well as company policy for quality and privacy of client information. Follows through with Pathways’ mandatory continuing education units annually.

    Set up client’s new home such as arranging lease documentation, arranging utility hook-ups or acquisition of furnishings and household goods.

    See clients face-to-face in their living environment weekly or as recommended via evaluation and as recommended by supervisor and team member input to maintain community living and avoid future hospitalization or decompensation.

    Timely, accurate and completed documentation of evals, plans, updates, closure summaries and daily progress notes are turned in weekly.

    Ongoing collaboration with client, family members, PCP, treatment teams, DSHS and residential staff and others involved in client care and documentation of such.

    Attend weekly staff/supervision meetings with Pathways Team. May attend monthly staff meetings with DSHS team members as required or requested.

    Participates in interdisciplinary treatment planning.

    Participates in program development and staff in-services.

    Performs administrative functions consistent with the needs of the program.

    Conducts interviews to assist individual to gain insight into personal problems, define goals, and plan action reflecting interest, abilities, and needs.

    Documents in medical record according to requirements of company and regulatory agencies.

    Evaluates data to identify causes of problem of individuals and to determine advisability of counseling or may refer to other specialist or institutions.

    Follows up results of services individual received to determine reliability and validity of services used.

    May be required to provide a minimum number of hours weekly, as specified by supervisor or director.

    May provide consultation/education to community service clubs, organizations, residential facilities, human service agencies, hospitals, and various county departments.

    May provide linkages to intensive interventions to develop skills aimed at mediating the effects of mental crisis, trauma, or depression.

    May be required to spend up to 70% of time providing direct service.

    Other duties as assigned or necessary to support the client, program and the company.

    Education/Licensing Requirements/or experience

    Bachelor’s degree in social work or psychology

    Two years’ experience in the coordination or provision of independent living services (ILS)

    Or two years’ experience in the coordination of ILS in a social service setting under qualified supervision

    Or four years personal experience with a disability

    Be able to pass state and federal background checks

    Have reliable vehicle with active car insurance

    Ability to perform basic word processing tasks and utilizes proprietary software, including experience in excel, for the completion of tasks or has the ability to learn these skills.

    Knowledge, Skills and Experience

    Minimum of two years experience with behavioral health patients (inpatient and/or outpatient).

    Must have direct case management or independent living skills experience and knowledge regarding rehabilitation philosophy.

    Strong interpersonal skills, and excellent written and verbal skills.

    Ability to perform basic word processing tasks and utilizes proprietary software for the completion of tasks or has the ability to learn these skills.

    Intermediate PC skills required.

    Must have experience and skills that are informed by advanced training, years of experience, and mastery of a range of competencies that demonstrate the following:Capacity to provide services that incorporate best practice,Ability to practice skills with team members.
