Job Summary
The Patient Care Coordinator assists the Case Management, Authorizations and Specialty Clinic Teams with the coordination of care between patients and members of the healthcare team. Reporting directly to the Clinic Manager, the Patient Care Coordinator helps Case Managers in overseeing their patient appointments, carrying out orders, and following up to ensure patients receive care in a timely manner. The Patient Care Coordinator also assists in the care of patients part of the Specialty Clinics such as SWAG+, Women’s Bleeding, Orthopedics, and MCAS Clinics. The Patient Care Coordinator helps to coordinate orders as directed by Physicians, creates and maintains a tracking system for appointments and due dates for future labs, follows up on referrals and imaging, and obtains records. The Patient Care Coordinator may assist in organizing care with the Gene Therapy Team from initial stages of eligibility testing to tracking patient progress post gene therapy treatment. This role will also be responsible for the preparation of weekly physician meetings to review patients and necessary follow up from each team member to ensure multidisciplinary team members are up to date on the patient’s plan of care and tasks. In order to be successful in this role, the Patient Care Coordinator must have the ability to multi-task and rapidly shift focus, have strong organizational skills with attention to detail, be customer service oriented with a positive and friendly disposition and have a proven ability to work both autonomously and as a team member.

Essential Job Duties
Treatment Plan Coordination: Assist Case Managers with treatment plan coordination by faxing orders and necessary documents to other institutions, entering labs/imaging/referrals as ordered by Physicians, notifying Authorizations as needed for insurance approval, and sending discharge summaries or other PA/MD paperwork to patients. Assist Case Manager post-MD visit by ensuring plan of care is completed or provide assistance as needed and act as a liaison between patients/Case Managers/Physicians to ensure continuity of care. Treatment plan coordination includes care for Specialty Clinics (e.g., MCAS; SWAG+; Women’s Bleeding; Orthopedics; MCAS).
Communication of Care : Assist Case Managers and the Authorizations Team in communicating plan of care to patients, working with Infusion Centers/Hospitals/Other Facilities to schedule appointments for patients, tracking patient infusion/transfusion schedules, and helping patients make appointments accordingly.
Administrative Support: Prepare meeting notes for Comprehensive Care and CCS meetings.
Required Education/Experience
High School Graduate or equivalent (advanced degree ideal)
2 years of experience in healthcare environment (outpatient community clinic environment ideal)
1 year of experience with Electronic Medical Records (EMR) system

Strong grasp of federal and state laws and ethical issues pertaining to PHI confidentiality and HIPAA requirements

The Center for Comprehensive Care & Diagnosis of Inherited Blood Disorders (CIBD), a 501(c)3 non-profit organization, is committed to providing comprehensive care for patients and their families with rare, chronic inherited blood disorders. CIBD is a part of a nationwide network of over 140 Hemophilia Treatment Centers that use multidisciplinary teams of blood disorder specialists to provide expert comprehensive diagnosis, medical care, outreach, education, outcomes monitoring, surveillance, and prevention services to people with Hemophilia, Sickle-Cell Anemia, Thalassemia, Von Willebrand Disease, Mast Cell Activation Syndrome (MCAS) and other related benign hematology conditions.
