    Position Overview

    The Patient Care Coordinator (PCC) is a cornerstone role within Coastal MD™, combining exceptional patient experience with education-driven service alignment. This elevated position builds upon our established Patient Experience framework while integrating key elements of our Vitality Coordinator model — including proactive education, relationship-building, and value-based discussions around services and memberships.

    Core Responsibilities

    Patient Experience & Engagement

    Serve as a primary point of contact for in-office, phone, and digital patient interactions

    Create a warm, professional, and seamless experience for every visit

    Proactively educate patients on Coastal MD™ services, programs, and memberships

    Discuss care options, including out-of-pocket and direct-pay services, with confidence and clarity

    Ensure patients feel supported, informed, and valued throughout their care journey

    Patient Education & Service Alignment

    Act as a knowledgeable guide, helping patients understand how services align with their health goals

    Support patient retention and engagement through trust-based, non-pressured conversations

    Model best practices in patient communication and service education

    Collaborate with leadership and marketing teams by providing real-time patient insights

    Front Desk & Administrative Operations

    Schedule appointments accurately across providers and locations

    Process payments, memberships, packages, and promotions

    Maintain detailed and accurate documentation within the EMR

    Manage incoming calls, messages, and digital correspondence

    Anticipate workflow needs and proactively support clinic operations

    Communication & Interpersonal Excellence

    Demonstrate strong verbal and written communication skills

    Maintain a calm, professional, and welcoming demeanor at all times

    Address patient concerns with empathy, discretion, and solution-focused thinking

    Navigate sensitive or complex conversations with confidence and emotional intelligence

    Team Collaboration & Culture

    Participate in team meetings, training sessions, and process improvements

    Support team members through collaboration and cross-coverage as needed

    Contribute to a positive, patient-centered workplace culture

    Flexibility & Multi-Location Support

    Coastal MD™ operates across multiple Long Island locations

    PCCs may be asked to support different offices based on scheduling needs

    Coordination is managed by the Director of Patient Experience to ensure balance and consistency

    Represent Coastal MD™ with professionalism in all interactions

    Maintain a polished appearance and professional demeanor

    Uphold strict HIPAA compliance and confidentiality standards

    Protect all patient and company information as proprietary

    Technology & Learning Agility

    Proficiency in EMR systems, scheduling platforms, and payment processing

    Comfortable using phone systems, Google Workspace, and the Coastal MD™ App

    Adaptable to evolving workflows and new technology

    Demonstrates strong learning agility and attention to detail
