    Patient Care Coordinator (Medical Assistant)

    Full-Time | San Diego/ Sorrento Valley Office + Local Senior Living Communities
    Schedule: Monday–Friday, 8:00 AM–5:00 PM (one-hour lunch)
    Compensation: $25–$28/hour based on experience

    We are a geriatric mobile medical practice that provides primary care to seniors in their homes and senior living communities across San Diego. We are looking for a Patient Care Coordinator who is a Certified Medical Assistant (MA) and who brings compassion, organization, and strong communication to their work.

    This role is primarily office-based. We do not see patients in the office—our providers visit patients in their homes and senior living communities. Your main work will be coordinating care by phone, fax, and EMR. 1–2 days per week , you will go onsite to one of our partnered senior living communities to support the provider. On those days, you will help greet and coordinate patients, communicate with nursing staff, follow up on orders, and ensure residents receive timely care.

    Key Responsibilities:

    Care Coordination & Scheduling

    • Maintain and organize provider schedules to ensure full and efficient visit days
    • Schedule new and follow-up appointments across multiple senior living communities
    • Register new patients and verify insurance and demographic information
    • Process medication refills, lab and imaging orders, referrals, and physician messages
    • Track lab and imaging results, confirm completion, and follow up for provider review

    Communication & Patient/Family Support

    • Communicate provider recommendations clearly and supportively
    • Serve as a steady point of contact to help patients and families understand care plans
    • Maintain ongoing check-ins with high-risk patients to prevent care gaps
    • Answer and triage phone calls from patients, families, community staff, and providers

    Care Transitions & Follow-Up

    • Track patients during hospital and skilled nursing stays
    • Notify and support families during transitions
    • Support next steps to ensure safe discharge to home or assisted living
    • Schedule post-hospital follow-up visits and assist in arranging home health and other services

    Onsite Clinic Support (1–2 Days/Week)

    • Work onsite at partner senior living communities to support the provider and residents
    • Greet patients, review needs with nursing staff, follow up on orders and care plans
    • Represent the company in a warm, professional, service-based manner
    • Build positive relationships with community staff and residents

    Documentation & EMR

    • Work efficiently in the EMR to document communication and update patient records
    • Maintain accurate patient lists, care coordination notes, and follow-up tasks
    • Epic experience is a bonus, but not required (we will train)

    Required Skills & Qualities

    • Certified/Registered Medical Assistant (MA)
    • Kind, patient, and comfortable working with seniors and family caregivers
    • Clear, calm communicator—especially when situations are emotional or complex
    • Organized and reliable with strong follow-through
    • Able to adjust plans throughout the day without losing focus
    • Curious, willing to learn, willing to ask questions
    • Able to build trust with patients, families, and community staff

    Experience

    • 1+ years in a medical office preferred
    • Experience in senior living community or mobile medical setting is a bonus
    • Experience coordinating care or managing multiple patient workflows is helpful
    • Experience in senior living communities or with geriatric patients is a strong plus
    • Epic EHR experience is a bonus

    Compensation & Schedule

    • $25–$28/hour depending on experience
    • Monday–Friday, 8 AM–5 PM
    • Office-based most days; onsite community support 1–2 days weekly
