About the Role:

Join LEV MONITORING SOLUTIONS INC as a Patient Care Liaison in the vibrant Northern Indiana area! In this pivotal role, you will connect patients with essential healthcare services, ensuring a seamless experience while representing a company dedicated to innovative monitoring solutions.

Responsibilities:

Act as the primary point of contact for patients, addressing inquiries and providing support.

Coordinate patient appointments and follow-up consultations to ensure continuity of care.

Educate patients on monitoring services and equipment usage for optimal health outcomes.

Build and maintain strong relationships with healthcare providers and community organizations.

Document patient interactions and maintain accurate records in compliance with HIPAA regulations.

Collaborate with internal teams to enhance patient care processes and service delivery.

Conduct outreach initiatives to engage and inform potential patients about services offered.

Stay updated on industry trends and best practices to continually improve patient experiences.

Requirements:

Proven experience in patient care or healthcare liaison roles 3-5 years

Strong communication and interpersonal skills to foster relationships.

Knowledge of healthcare regulations and patient privacy standards.

Ability to work independently and as part of a collaborative team.

Compassionate demeanor with a genuine interest in patient welfare.

Valid driver’s license and willingness to travel within the Northern Indiana and surrounding areas.

Relevant certifications in healthcare or patient advocacy are a plus.

Cardiology experience preferred
