Qualifications:
- Education: Bachelor of Science in Nursing (BSN) preferred.
- Licensure: Current and unencumbered Registered Nurse (RN) license in the state of Practice.
- Experience: A minimum of two years' experience in a management or supervisory role within a home care or home health setting is highly preferred.
- Technical Skills: Strong knowledge of professional standards, Medicare Conditions of Participation, and federal, state, and local regulations.
- Soft Skills: Except for leadership, mentorship, communication, and interpersonal skills. Ability to serve as a liaison between patients, families, and staff.
Key Responsibilities:
- Manage and oversee patient care services, ensuring compliance with all regulatory standards.
- Lead, mentor, and support nursing staff, fostering professional growth and enhanced patient outcomes.
- Collaborate with interdisciplinary teams to develop and implement individualized patient care plans.
- Ensure accurate and timely documentation of patient care activities and outcomes.
- Conduct regular audits of patient records and care plans to uphold quality assurance.
- Act as a liaison among patients, families, and healthcare providers to ensure seamless care coordination.
- Engage in quality improvement initiatives and participate in ongoing professional development.
