The Patient Care Manager plays a crucial role in facilitating the progression of care and closely monitors the patient's journey to guarantee that the care plan and services delivered are centered around the patient, uphold high quality, are evidence-based, tailored to the patient's needs, efficient, and cost-effective.

Qualifications

_ What’s Required_

• A Bachelor’s degree in Nursing is required.
• Equivalent experience may be accepted in place of a Bachelor’s degree in nursing (an RN license is mandatory).
