The Remote Patient Navigation Assistant supports patients as they move through the healthcare system by providing virtual guidance, education, and care coordination. This role helps patients understand their care plans, access services, and overcome barriers to care, all through phone, video, and digital communication. The position works closely with clinical teams, administrative staff, and community resources to ensure a seamless, patient-centered experience.
Key Responsibilities
Serve as a primary virtual point of contact for patients via phone, video, email, and secure messaging
Assist patients with appointment scheduling, referrals, and follow-up coordination
Educate patients on care plans, procedures, medications, insurance coverage, and available resources
Identify and address barriers to care such as transportation, financial challenges, technology access, language needs, or health literacy
Coordinate with physicians, nurses, care managers, social workers, and external providers to ensure continuity of care
Support patients with completing forms, consents, and other documentation electronically
Track patient progress, conduct follow-up outreach, and reinforce adherence to care plans
Accurately document patient interactions and updates in electronic health records or care management platforms
Provide compassionate, culturally sensitive support to patients and caregivers
Maintain compliance with HIPAA, privacy, and organizational policies while working remotely
Qualifications
High school diploma or equivalent required; associate or bachelor’s degree in healthcare, public health, social work, or a related field preferred
Previous experience in healthcare, patient navigation, care coordination, call center, or customer support roles preferred
Familiarity with healthcare systems, insurance processes, and community resources is a plus
Strong verbal and written communication skills
Ability to explain complex information clearly in a virtual environment
Excellent organizational skills and ability to manage multiple patients remotely
Proficiency with electronic health records, telehealth platforms, and standard office software
Reliable internet connection and a private, secure workspace
