Must have 3+ years working in a social services environment preferrable the non-profit sector. Must be able to manage and coordinate client needs; perform a variety of duties involved in assessing, planning, implementing and evaluating services provided to cancer patients. Must be able to provide emotional support and counseling to patients and their families. Prefer experience working with the elderly. Bilingual preferred.
Work Requirements:
Act as patient advocate; identify and develop new community resources; assist with problem solving
Assisting patients in understanding their diagnosis, treatment options, and the care process
Coordinating appointments with doctors and other healthcare professionals
Ensuring patients have access to the medical treatments or services they need
Helping patients understand and complete necessary paperwork
Educating patients about resources and support services available
Acting as a liaison between patients and healthcare professionals including doctor visits with patients
Coordinating community outreach group meetings
Helping patients navigate through the healthcare system, including insurance issues
Provide referrals to appropriate community resources; monitor activities to ensure that services are actually being delivered
Acts as the clients’ formal and informal support systems
Maintain accurate patient records; maintain patient confidentiality
Prepare clear and concise reports
Assess the physical, functional, social, mental, environmental, and financial needs of the client.
Attend and coordinate health fairs and other community outreach services
