Position Overview:
We are seeking a warm, patient-focused individual to serve as a Patient Relations Coordinator for a growing homecare agency. This critical role ensures new and existing patients feel supported, heard, and fully satisfied with their care. The coordinator will serve as the first point of contact for patient concerns, following up on new placements and maintaining relationships with long-term clients.
Key Responsibilities:
Contact new patients after services begin to ensure they’re satisfied with their care and assigned aides
Serve as a support line for existing patients with concerns or complaints
Document all communications and follow-up in a timely and organized manner
Collaborate with the clinical and coordination teams to resolve issues and ensure continuity of care
Maintain an ongoing, respectful relationship with patients and family members
Monitor trends in feedback to identify areas of improvement across the agency
Ideal Candidate & Qualifications
Experience in patient-facing roles within healthcare is preferred
Strong phone etiquette and interpersonal skills
Warm, empathetic, and solutions-oriented personality
Ability to manage multiple conversations with calm and clarity
Proactive in identifying and resolving patient issues
