Responsibilities:
Work with the patient and the family to design and implement an individualized plan of care
Update progress notes and other documentation on an ongoing basis
Collaborate with other team members, when applicable, in the performance of patient care
Requirements and Qualifications:
Master's or Doctorate of Physical Therapy from an accredited program
IL PT licensure
Passion for pediatric patients
Desire to partner alongside parents
Motivated and eager to learn
Excellent communication skills
Collaborative, team-building personality style
