Per Diem Nurse Practitioner **
Location** : Alhambra, CA 91803. **
Job Type:** Per Diem, 1099 Contract. **
Pay Rate:** $130/ hr for onsite patient care, $100/ hr for virtual lab review. **
Schedule:**
Alhambra: one day either 7/28 or 7/30, Dayshift.
Anaheim: TDB Weekday, Dayshift.
Riverside: TDB Weekday, Dayshift.

VitalCheck Wellness is seeking a Nurse Practitioner to provide general wellness checkups on site for employees of a private business with locations throughout Los Angeles, CA. Our office arranges the patient appointments and does the administrative work, giving you the time and luxury to provide clinical one-on-one patient care (usually no more than 20 patients per day).

Responsibilities:

• Provide general wellness checkups.
• Consult with patients to understand their health concerns.
• Advise/ refer patients for follow-up care.
• Perform blood draws for patients.
• Provide telemedicine services for lab review follow-ups.

Requirements:

• Active clean Nurse Practitioner license to practice in __ California.
• Family Nurse Practitioner (FNP)/ (FNP-C)/ (FNP-BC)/ Acute Care Nurse Practitioner (ACNP) certified.
• Active BLS/ ACLS certification required.
• A collaborating physician is necessary unless state requirements for independent/autonomous practice are met.
• Background in internal medicine, family practice, urgent care, or emergency room preferred.
• Must be able to perform phlebotomy.
• Proficient with computers, customer service-oriented, and able to work independently.
• Focused on preventive medicine and patient education.
• Personal laptop or tablet required.
• Must be able to reliably commute throughout Los Angeles, CA.
