    Position Overview

    Community Health Net is seeking a dedicated and knowledgeable Per-Diem Pharmacist to support our pharmacy team in delivering safe, effective, and patient-centered care. The Pharmacist dispenses prescription medications, provides counseling to ensure proper medication use, and monitors patient outcomes to support optimal therapy. This role plays an essential part in advancing our mission to provide exceptional care to all members of the community, including underserved populations.

    Key Responsibilities

    • Accurately compound and dispense prescription medications in accordance with physician orders, legal requirements, and professional standards.

    • Provide clear, patient-centered counseling regarding medication use, side effects, interactions, and proper storage.

    • Ensure patients understand the instructions and guidance provided by prescribers and other healthcare practitioners.

    • Monitor patient health and therapeutic progress to ensure medications are safe and effective.

    • Maintain compliance with Pennsylvania drug laws, controlled substance regulations, and Community Health Net policies.

    • Collaborate effectively with physicians, clinical staff, and team members to support coordinated patient care.

    • Present a professional, courteous, and compassionate demeanor when interacting with patients, referral sources, and the public.

    • Maintain accurate records and perform related duties to ensure efficient pharmacy operations.

    Requirements

    • Current Pennsylvania Pharmacist License and controlled substance registration required.

    • Thorough knowledge of:

      • Principles of dispensing prescriptions

      • Dosage calculations and preparation/use of percentage and stock solutions

      • Pharmaceutics, medications, narcotics, and poisons

      • Applicable Pennsylvania pharmacy and drug laws

    • Retail and/or clinical pharmacy experience preferred.

    • Strong analytical and decision-making skills with the ability to apply sound judgment.

    • Excellent communication skills and the ability to convey information clearly and professionally.

    • Demonstrated ability to work effectively in a collaborative, team-oriented environment.
