Per Diem Physician (MD/DO) **
Location:** Johnstown, NY. **
Job Type:** Per Diem Independent Contractor, 1099 Contract. **
Pay Rate:** $180/hr for onsite patient care, $140/hr for virtual lab review. **
Schedule:** This is a 3-day onsite, day-shift clinic taking place on Tuesday, Wednesday, and Thursday in May (exact dates TBD).
Employer: A New York physician-owned professional corporation (the “PC”).

Full Description:
VitalCheck Wellness, as the administrative agent, is seeking, on behalf of a New York professional corporation owned and directed by licensed physicians (PC), is seeking a Physician to provide onsite general wellness checkups and participate via telemedicine in a structured multi-week treatment and coaching program for Chronic Metabolic Diseases (i.e Pre-diabetes, Hypertension, Hyperlipidemia, Obesity) with specific exclusion of Diabetes, designed to improve their wellness and lifestyle.

We handle all patient appointments and administrative work, allowing you to focus entirely on one-on-one clinical patient care. This is an excellent opportunity to practice preventive and personalized medicine in a supportive, structured setting. The Physician will be employed by the PC.

Responsibilities:

• Provide general wellness checkups.
• Consult with patients to understand their health concerns.
• Advise/refer patients for follow-up care.
• Provide telemedicine services for lab review follow-ups.
• Provide chronic disease management to patients based on our program guideline.
• Educate patients around healthy eating and lifestyle.
• Prescribing chronic disease management medications.

Requirements:

• MD or DO degree.
• Clean active license to practice in the state of New York.
• Active BLS/ ACLS certification required.
• Board certification in specialty.
• Minimum 3-5 years of work experience following post graduate training.
• Background in family practice and preventive care preferred.
• Background in urgent care or emergency care will be considered.
• Focused on preventive medicine and patient education.
• Proficient with computers, customer service-oriented, and able to work independently.
• Personal laptop or tablet required.
• Must be able to reliably commute to Johnstown, NY.
