Responsibilities:

· Initial assessments & reassessments

· Develop/update Plans of Care

· Conduct caregiver supervisory visits

· Skilled nursing visits as ordered

· Ensure NJ compliance & timely documentation

Requirements:

· Active NJ RN License (required)

· 2+ years clinical experience (home care preferred)

· Reliable transportation

· Strong documentation & communication skills

Schedule:

Per diem / As needed Evenings or weekends a plus
