    Job Summary:

    As a PAS Supervisor at Hoping Hearts Home Care, you play a vital leadership role in promoting

    communication and collaboration among owners, staff, caregivers, and the community. You are

    responsible for coordinating caregiver operations, managing administrative functions, and ensuring

    the delivery of high-quality, client-centered care. This role supports the agency’s strategic,

    operational, and financial goals while fostering a positive and compliant home care environment.

    Key Responsibilities:

    Care Coordination & Supervision

    Coordinate services, schedule client visits, and supervise field staff.

    Conduct initial home care visits, re-evaluate client progress, and notify physicians of any

    changes in condition.

    Monitor and oversee all client care and ensure documentation is completed in a timely and

    accurate manner.

    Maintain organized and confidential client records per agency policies and regulatory standards.

    Fill in during staff shortages to maintain continuity of care.

    Leadership & Team Collaboration

    Foster strong teamwork among caregivers, administrative staff, and leadership.

    Organize monthly caregiver meetings to share updates, gather feedback, and address concerns.

    Distribute and analyze quarterly caregiver surveys to identify areas for improvement.

    Facilitate onboarding and training of new caregivers, ensuring they understand company

    policies and expectations.

    Answer work-related calls promptly to support staff, clients, and families.

    Administrative & Compliance Oversight

    Conduct regular audits of client files for accuracy and regulatory compliance.

    Manage hospitalization and discharge records to ensure proper documentation.

    Assist in filing and tracking incident reports and other required documentation.

    Follow and enforce infection control procedures in both office and field settings.

    Participate in the on-call rotation and support the agency’s quality improvement initiatives.

    Ensure public information, communications, and materials are accurate and aligned with agency

    standards.

    Monitor personnel availability and qualifications in line with care plan requirements.

    Program Development & Performance Improvement

    Develop and implement department activities such as workshops, caregiver education, and

    community outreach.

    Create and revise care-focused policies in coordination with Owners/Officers and care staff.

    Support performance improvement priorities and ensure best practices in client care.

    Monitor outcomes to ensure care delivery aligns with client needs and agency standards.

    Qualifications and Requirements

    Education & Experience (Must meet one of the following):

    High school diploma or GED with one year of experience caring for individuals with functional

    disabilities; or

    Completion of two years of full-time study at an accredited college/university in a health-related

    field; or

    Licensure as a physician, registered nurse (RN), social worker, therapist, or nursing home

    administrator with at least one year of management or supervisory experience in a health-

    related setting.

    Additional Requirements:

    Strong communication and interpersonal skills.

    Excellent organizational and multitasking abilities.

    Proficient in administrative duties, documentation, and client recordkeeping.

    Ability to lead a team and foster a collaborative work environment.

    Completion of a minimum of 12 clock hours of continuing education per year in relevant

    subjects.

    Work Conditions

    Office and field-based role with regular client visits in Austin and surrounding counties.

    Participation in after-hours on-call rotation.

    Occasional attendance at conferences, workshops, and training events.
