Job Description:
We are seeking a pharmacist with a strong background in managing common chronic disease states such as diabetes, hypertension, and hyperlipidemia, other common disease states. The ideal candidate will have a solid understanding of current clinical guidelines (e.g., ADA, ACC/AHA, AACE) and experience delivering Medication Therapy Management (MTM) services to improve patient outcomes.
As a Pharmacist, you will be receiving incoming prescriptions, reviewing them for accuracy, and filling them with the correct medicine and dose. You will also be talking with customers to help them understand their medications and possible side effects, advising them on medication storage, and maintaining a safe, clean work environment. The ideal Pharmacist candidate is comfortable working in a fast-paced environment, has excellent communication and interpersonal skills, and is familiar with pharmaceutical best practices.

Key Responsibilities:
Conduct MTM sessions with patients, focusing on chronic conditions and medication adherence.
Stay current with evolving clinical guidelines and apply evidence-based practices in patient care.
Collaborate with physicians and other healthcare professionals to optimize medication therapy.
Educate patients on disease state management, medication use, and lifestyle modifications.
Identify and resolve drug-related problems to enhance therapeutic outcomes
Vaccine administration.
Retail pharmacy operations- Read and review incoming prescriptions for accuracy, suitability to patient, and potential interactions,, provide information to patients on side effects, dosage, specific storage needs, and interactions with other medications
Maintain excellent patient records and pharmacy files
Conduct regular inventory and maintain proper standards for accepting new inventory and categorizing it appropriately
Qualifications:
• PharmD degree (required)
• Valid pharmacist license in [California]
• Minimum 1-2 years of clinical care preferred or retail pharmacy experience.
• Familiarity with guidelines from ADA, ACC/AHA, AACE, and others
• MTM experience required (Outcomes MTM or similar platforms preferred)
• Excellent communication and patient counseling skills
Preferred Experience:
• Chronic disease management (DM, HTN, HLD)
• EMR documentation making SOAP notes
• Collaborative practice or working with provider teams
Retail pharmacy experience.
