About the Role
Join a high-impact California Population Health Team dedicated to improving medication adherence and closing healthcare quality gaps. In this fully remote role, you will support key initiatives including Medication Adherence, Statin Therapy, and HEDIS/STARS Part D quality measures.
As a Pharmacy Care Technician, you will work in a fast-paced, outbound call center environment engaging health plan members to improve medication access, remove barriers to adherence, and coordinate care with providers and pharmacies. You will report directly to the Director of Population Health and collaborate with advanced practice clinicians, outreach coordinators, and pharmacy professionals to drive measurable healthcare outcomes.
Key Responsibilities
Conduct outbound member outreach calls focused on medication adherence and quality gap closure (HEDIS/STARS Part D)
Identify barriers to medication use and provide solutions or escalate concerns appropriately
Coordinate with prescribers, provider offices, and pharmacies to resolve refill and access issues
Process refill requests and assist with prior authorization communication within scope
Review medication profiles to identify adherence opportunities and quality metric impact
Accurately document all outreach efforts and resolutions in real time
Monitor and follow up on open medication-related tasks to ensure timely completion
Maintain productivity and quality benchmarks in a remote call center setting
Ensure full compliance with HIPAA and regulatory standards
Participate in team meetings, training, and performance improvement initiatives
Required Qualifications
Active Pharmacy Technician license in state of residence
At least 1 year of experience working remotely in a cross-functional team environment
Strong ability to work independently with excellent organizational skills
Experience navigating multiple systems and databases simultaneously
Intermediate proficiency in Microsoft Word, Excel, and Outlook
Ability to work 8:00 AM – 4:30 PM PST, Monday through Friday
Must be 18 years of age or older
Preferred Qualifications
Prior call center experience (inbound and/or outbound)
National Pharmacy Technician Certification (PTCB or NHA)
Experience in managed care, PBM, or health plan environment
Bilingual skills (Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese), especially with medical terminology
