Job Summary
We are seeking a detail-oriented and motivated Pharmacy Technician to support our Chronic Care Management (CCM) program by performing accurate medication reconciliations, ensuring compliance with Centers for Medicare & Medicaid Services (CMS) requirements. The ideal candidate will work closely with pharmacists, providers, and care coordinators to identify discrepancies, prevent adverse drug interactions, and optimize medication regimens for Medicare patients enrolled in CCM.
Responsibilities
Conduct thorough medication reconciliations for CCM patients by reviewing prescriptions, over-the-counter medications, supplements, and patient-reported usage.
Contact patients, pharmacies, and providers as needed to clarify medication lists and usage patterns.
Document all medication information and updates in the electronic health record (EHR) accurately and in accordance with CMS guidelines.
Identify and report potential drug interactions, duplications, or safety concerns to the supervising pharmacist or clinical provider.
Assist in maintaining up-to-date medication lists that support CCM billing and compliance documentation.
Track and ensure annual wellness visit (AWV) and chronic condition follow-up documentation aligns with medication reconciliation efforts.
Participate in care team meetings to provide updates on medication status and patient adherence.
Assist with patient education on proper medication use and adherence strategies.
Support the CCM team in meeting CMS quality measures and audit-readiness.
Maintain patient confidentiality and comply with HIPAA regulations at all times.
Qualifications
High school diploma or equivalent required
Certified Pharmacy Technician (CPhT) credential required (depending on state regulations).
Prior experience with medication reconciliation, chronic care management, or a clinical pharmacy setting strongly preferred.
Proficient with EHR systems and comfortable using healthcare technology.
Strong knowledge of pharmacology, drug interactions, and CMS guidelines related to CCM.
Excellent communication, organizational, documentation, and strong problem solving skills.
Ability to work independently and collaboratively in a fast-paced clinical environment.
Preferred Skills: Experience with Medicare billing or CMS audit preparation.
Familiarity with clinical terminology and chronic disease states.
