Job Description
The RN On-Call is a valuable member of the hospice team and will be responsible for meeting the needs of our patients outside normal business hours. This position will return all calls received outside of office hours; working as part of the interdisciplinary team to provide a holistic approach to end of life care and symptom management.
DUTIES AND RESPONSIBILITIES
- Coordinates patient/family care to support terminally ill patients and families in home, skilled nursing facility or residential care facility.
- Assess home care needs, being aware of the physical, emotional, and spiritual aspects as well as gather data on social, economic and cultural factors which may influence health, well-being and quality of life
- Prepare clinical notes and other required documentation within required time-frames.
- Consult with primary care provider, and other health professionals as required.
- Develop and maintain rapport with patient and families and work well with an interdisciplinary team
- Identify patient/family goals and evaluate the patient’s response to care.
- Provides care according to plan of care.
- Provide end of life nursing expertise, advocacy, resource linkage as needed.

EDUCATION:
- Graduate of an approved School of Professional Nursing;
- Current state RN licensure in the state of practice.
- Possess and maintain current CPR certification.
EXPERIENCE:
- Experience working as an RN, preferably in Hospice, Palliative Care, or Oncology preferred
- Minimum one (1) to two (2) years’ recent experience in home care preferred
