About the Role:
ADDAX HOME HEALTH LLC is looking for PRN passionate and skilled Physical Therapists/Physical Therapists Assistants to join our growing team in Fort Myers, FL. This is an exciting opportunity to make a meaningful impact by delivering high-quality, patient-centered care directly in the comfort of patients' homes. Responsibilities:
Conduct comprehensive physical therapy evaluations and assessments in patients' homes
Develop and implement individualized treatment plans to restore function and mobility
Provide therapeutic exercises, manual therapy, and rehabilitative interventions
Monitor and document patient progress, adjusting care plans as needed
Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
Educate patients and caregivers on home exercise programs and injury prevention
Maintain accurate and timely clinical documentation in compliance with regulatory standards
Requirements:
Valid Florida Physical Therapist (PT) license in good standing
Graduate of an accredited Physical Therapy program (DPT or MPT)
Home health experience preferred
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills with patients and care teams
Reliable transportation and valid driver's license for home visits in the Fort Myers, Cape Coral, Lehigh Acres, Alva, Labelle, Punta Gorda, Estero, Bonita Beach area
CPR/BLS certification required
