Key Responsibilities

• Conduct comprehensive initial patient evaluations and assessments in acute care rehabilitation settings
• Develop and implement individualized evidence-based treatment plans for diverse patient populations
• Administer therapeutic interventions including manual therapy, therapeutic exercises, and modalities (ultrasound, electrical stimulation, heat/cold therapy)
• Provide patient and family education on exercise programs, injury prevention, posture correction, and home care strategies
• Collaborate with multidisciplinary healthcare teams including physicians, nurses, occupational therapists, and case managers
• Monitor and document patient progress, adjusting treatment protocols to optimize rehabilitation outcomes
• Maintain accurate clinical documentation including evaluations, progress notes, and discharge summaries in compliance with regulatory standards
• Deliver compassionate, patient-centered care for individuals with orthopedic, neurological, and musculoskeletal conditions
• Ensure adherence to infection control protocols and maintain a safe, organized treatment environment
• Participate in quality improvement initiatives and continuing education opportunities

Required Qualifications

• Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program
• Current New York State Physical Therapist license or eligibility for licensure at time of hire
• Basic Life Support (BLS) certification required within 90 days of employment
• Prior experience in acute care, inpatient, or outpatient physical therapy settings strongly preferred
• Excellent verbal and written communication skills for effective patient and team collaboration
• Strong clinical reasoning, critical thinking, and problem-solving abilities
• Exceptional organizational skills and time management capabilities
• Ability to lift up to 30 pounds independently and up to 150 pounds with assistance
