Job Overview: Provides in-home physical therapy support for adult and geriatric patients. Home visits and periodic office meetings required. Must have reliable transportation, a current driver’s license, and agency-required liability insurance. Average 5–6 patients per day performing start-of-care and follow-up visits.

Responsibilities:

• Assist with patient assessments and implement physical therapy plans under the direction of a licensed PT.
• Document patient progress, including OASIS, evaluations, reassessments, discharge planning, and short- and long-term goals.
• Provide patient and family education to support independence and functional outcomes.
• Apply time management to meet daily productivity expectations.
• Collaborate with the interdisciplinary team to ensure coordinated care.

Requirements:

• Minimum 2 years of experience in Home Health or related outpatient/home therapy settings.
• PTA license required.
• CPR/BLS certification required.
• Proficiency in HomeCare HomeBase, OASIS, and Outlook EMR systems.
• Must be willing to travel up to 75 miles from home clinic.
• First-time home health PTAs not accepted; recent home health experience required.
