About Qualicare and the Role
At Qualicare Home Health, we pride ourselves on providing patient-centered care that promotes recovery and independence at home. We are seeking a motivated Physical Therapist Assistant (PTA) to join our growing team in San Ramon. Working under the supervision of a licensed PT, you will deliver individualized therapy treatments and assist clients in achieving their mobility and functional goals.
Key Responsibilities
Implement physical therapy programs established by the supervising PT
Assist patients with therapeutic exercises, mobility training, and safety education
Observe and report patient responses and progress to the PT
Reinforce patient and caregiver education regarding home exercises and adaptive equipment use
Document all treatments accurately and promptly in EMR (WellSky)
Communicate effectively with the care team to ensure quality outcomes
Follow established infection control and safety procedures during in-home visits
Qualifications
Active, unrestricted California PTA license
Associate degree from a CAPTE-accredited Physical Therapist Assistant program
Minimum 1 year of clinical experience (home health or outpatient preferred)
Current CPR certification
Reliable transportation and valid California driver’s license
Excellent communication, time management, and documentation skills
EMR proficiency preferred (WellSky or similar)
