Your Role

We’re seeking a compassionate, experienced Home Health Physical Therapist Assistant (PTA) to join our high-performing team. The ideal candidate has strong clinical judgment, outstanding communication skills, and a deep commitment to patient-centered care. You’ll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of home-based therapy.

What You’ll Do

· Provide skilled physical therapy treatments in patients’ homes under the direction of a licensed Physical Therapist

· Carry out individualized treatment plans focused on mobility, strength, balance, and functional independence

· Assist patients with therapeutic exercises, gait training, transfers, and mobility training

· Educate patients and caregivers on safe mobility techniques, home exercise programs, and fall prevention strategies

· Observe, document, and report patient responses and progress to the supervising Physical Therapist

· Communicate effectively with physicians, nurses, and other members of the interdisciplinary care team

· Maintain accurate, timely, and compliant clinical documentation Ensure compliance with all Medicare, CMS, and home health regulations

What We’re Looking For

· Current Florida Physical Therapist Assistant (PTA) license (required)

· Previous home health experience is strongly preferred but not required

· Excellent communication, time management, and documentation skills

· A compassionate, dependable, and professional attitude

· A team-oriented mindset and a genuine passion for making a difference in patients’ lives
