    The Physical Therapist Assistant (PTA) provides skilled physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist (PT), in accordance with the physician’s plan of care, Medicare Conditions of Participation, and ACHC accreditation standards. The PTA promotes patient independence, safety, and functional mobility while ensuring accurate, timely documentation and compliance with regulatory requirements.

    What you’ll be doing:

    Provide skilled physical therapy interventions in the patient’s home as delegated by the supervising Physical Therapist and outlined in the plan of care.

    Improve or minimize residual physical disabilities of the patient.

    Return the individual to optimum and productive level within the patient’s capability.

    Participate with all other home care personnel in patient care planning.

    Complete all visit documentation accurately, timely, and in compliance with Medicare, ACHC, and agency policies. Document services provided, patient tolerance, progress toward goals, and any education delivered during each visit.

    Graduate of an accredited Physical Therapist Assistant program. Current, active PTA license in the state of practice

    CPR/BLS certification (Healthcare Provider level)

    Valid driver’s license, reliable transportation, and current auto insurance

    Ability to pass background check, drug screening, and health clearance per agency policy

    Two (2) years’ experience preferred in a home health setting

    Knowledgeable and experience with Medicare OASIS documentation
