Job Summary
We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will provide high-quality patient care primarily within assisted living facilities. This role involves developing individualized treatment plans, implementing therapeutic interventions, and collaborating with other healthcare professionals to enhance patient recovery and mobility. The Physical Therapist will work with diverse patient populations, including geriatrics, those with physical limitations, and those recovering from orthopedic injuries.
Duties
- Assess patients' physical conditions and functional abilities through comprehensive evaluations.
- Develop and implement personalized care plans based on individual patient needs and goals.
- Utilize various therapeutic techniques, including trigger point therapy, to alleviate pain and improve mobility.
- Educate patients and their families about treatment options, exercises, and self-care strategies.
- Monitor patient progress and adjust treatment plans as necessary to ensure optimal recovery.
- Collaborate with interdisciplinary teams to coordinate care and discharge planning.
- Maintain accurate documentation of patient evaluations, treatments, and outcomes in compliance with healthcare regulations.
- Lead exercise classes and actively participate within the community.
- Stay current with advancements in physical therapy practices and techniques.
Experience
- A degree in Physical Therapy from an accredited program is required.
- Previous experience in a hospital or outpatient setting is preferred.
- Knowledge of anatomy and physiology is essential for effective patient assessment and treatment.
- Familiarity with various therapeutic modalities relevant to orthopedics, geriatrics, pediatrics, and discharge planning is highly desirable.
- Strong communication skills to effectively interact with patients, families, and healthcare team members.
