Physical Therapist – Home Health (Orthopedics & Discharges Focus)

The Role

We are seeking a skilled and patient-focused Physical Therapist to join a growing home health team. This role is primarily focused on orthopedic care and managing patient discharges. The ideal candidate will have experience in home health, a strong understanding of therapeutic best practices, and a commitment to helping patients regain independence and mobility in their own homes.

Compensation & Structure

• Salary Range: $100,000–$105,000

• EMR System: Kantime

• Weekly Point Expectation: 30 points

• Additional points above 30 paid at the per diem rate

• Primary Focus: Orthopedic cases and discharges

• Home Health Experience: Preferred

What You Will Do

• Conduct comprehensive physical therapy evaluations and develop personalized treatment plans to help patients meet recovery goals.

• Deliver direct therapy interventions aimed at improving mobility, balance, strength, and overall function.

• Educate patients and families on exercises and safe practices to support ongoing recovery at home.

• Monitor and document progress, adjusting treatment plans as needed, and communicate updates with the care team.

• Collaborate with physicians, nurses, and other healthcare providers to ensure integrated and effective patient care.

Who You Will Be

• Graduate of an accredited Physical Therapy program with a current Florida Physical Therapy license.

• At least 1 year of physical therapy experience, preferably in a home health setting.

• Proven ability to create individualized and effective treatment plans tailored to orthopedic recovery and discharge planning.
