Physical Therapist – Per Diem

We are seeking a licensed Physical Therapist (Per Diem) to join our growing healthcare team. This role is ideal for a self-motivated clinician looking for flexible, as-needed work while providing high-quality, patient-centered care.

Responsibilities:

Perform patient evaluations and reassessments

Develop and implement individualized treatment plans

Provide therapeutic exercises, manual therapy, and patient education

Monitor patient progress and modify care plans as needed

Complete accurate and timely clinical documentation

Communicate effectively with patients, families, and care team members

Comply with all healthcare, safety, and ethical standards

Qualifications:

Active Physical Therapist license

Ability to work independently and manage time efficiently

Strong communication and interpersonal skills

Reliable transportation (for home health visits, if applicable)

Home health experience preferred but not required

Position Details:

Job Type: Per Diem

Schedule: Flexible / As needed

Pay (Per Visit):

Physical Therapy Evaluation: $90 per evaluation

Physical Therapy Visit: $65–$75 per visit
