    As a Physical Therapist on our client’s Home Health team, you will provide skilled physical therapy services to patients in their homes. Responsibilities include evaluating functional status, creating individualized care plans, and supporting patients in improving independence and overall physical and cognitive function. The role also includes patient and caregiver education, appropriate supervision, and thorough documentation of patient progress. New graduates are welcome and will receive mentorship and support to ensure a successful transition into the role.

    Key Responsibilities

    Perform Physical Therapy assessments and develop treatment programs

    Record and report patient's reaction to treatment and condition changes

    Instruct patients and caregivers in therapy programs and device use

    Supervise physical therapy assistant and, when appropriate, the certified nursing assistant

    Prepare reports and participate in staff and Agency development activities

    We are looking for compassionate physical therapists with:

    Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required

    Licensed Physical Therapist issued by the state in which you will be working.

    Minimum of one year healthcare experience but open to new graduates

    Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

    A commitment to consistently meet critical deadlines for charting

    The skills needed to self-manage your time and schedule

    Demonstrated experience with tablets, mobile phones and EMR software

    PPV position-Rates $60+ depending on type of visit

    Annual salary averages $80K+
