Job Responsibilities

• Develop and implement individualized physical therapy treatment plans based on physician orders and patient evaluations
• Conduct comprehensive patient assessments and document detailed evaluations, progress notes, and discharge summaries
• Deliver skilled physical therapy interventions across outpatient rehab, skilled nursing facilities, and assisted living environments
• Collaborate with interdisciplinary teams including nursing staff, occupational therapists, and rehabilitation professionals to optimize patient outcomes
• Participate actively in care planning meetings and contribute clinical expertise to enhance residents' functional independence
• Maintain accurate treatment documentation, track patient statistics, and manage supply inventory requests
• Lead or participate in staff training, continuing education programs, and clinical mentorship initiatives
• Provide leadership support and clinical oversight in the absence of the Director of Physical Therapy

Job Requirements

• Graduation from an accredited Physical Therapy program (DPT or equivalent degree required)
• Active New York State Physical Therapy license in good standing
• Strong clinical interest in geriatric rehabilitation, subacute care, and adult patient populations

• Previous experience in skilled nursing, outpatient rehab, or post-acute care settings preferred but not required
• Demonstrated leadership abilities and capability to supervise physical therapy assistants and support staff
