Job Title: Physical Therapist – Home Health

Department: Home Health – Physical Therapy

Job Overview: Provides in-home physical therapy visits for adult and geriatric patients. Home visits and periodic office meetings required. Must have reliable transportation, a current driver’s license, and agency-required liability insurance. Average 5–6 patients per day, performing start-of-care and follow-up visits.

Responsibilities:

• Conduct assessments, develop and implement plans of care, and set short- and long-term therapy goals.
• Manage patient documentation including OASIS, evaluations, reassessments, discharge planning, and RUG levels.
• Provide patient/family education and support to promote independence and functional outcomes.
• Apply time management to meet daily productivity expectations.
• Collaborate with interdisciplinary teams to ensure coordinated care.

Requirements:

• Minimum 2 years of experience in Home Health Physical Therapy.
• Current PT license required.
• CPR/BLS certification required.
• Proficiency in HomeCare HomeBase, OASIS, and Outlook EMR systems.
• Must be willing to travel up to 75 miles from home clinic.
