Full-Time Physical Therapist (PT) – Home Health
Full-time · Field-based · Salem & Eugene, OR (multi-city territory)
Compensation: ~$100,000/year + up to $20,000 sign-on bonus (based on experience)
About the role
We’re working with a regional home health provider to hire a full-time Physical Therapist to support patients in their homes across Salem, McMinnville, and Eugene. You’ll design and deliver individualized treatment plans, educate patients and caregivers, and collaborate with an interdisciplinary team to help patients regain function, independence, and quality of life. This is a field-based role with daily travel to patient homes. New graduates are welcome to apply.
Responsibilities
Conduct comprehensive PT evaluations in the home and develop individualized treatment plans
Deliver therapeutic exercises, manual therapy, and functional mobility training to improve strength, balance, and independence
Educate patients and caregivers on safety, home-exercise programs, and long-term self-management strategies
Coordinate care with nurses, OTs, social workers, and physicians to support integrated, team-based care
Document patient progress, outcomes, and plan updates in the EMR within required timeframes
Maintain HIPAA compliance, safety standards, and equipment sanitation on every visit
Travel between patient homes within Salem, McMinnville, and Eugene; mileage reimbursement and flexible scheduling are provided
Eligible for a tiered sign-on bonus (range: $5,000–$20,000, based on years of experience)
Qualifications
Active Oregon Physical Therapist license (Doctorate or Master’s preferred)
Open to new graduates; prior home health experience is a plus
Current CPR/BLS certification
Reliable transportation with auto insurance and a valid Oregon driver’s license (daily travel to patient homes required)
Strong clinical reasoning, communication, and patient-education skills
Ability to work independently, manage a dynamic caseload, and adapt to changing patient needs
Ideal candidate profile
Licensed Oregon PT with at least 1 year of home-health experience or strong clinical rotations
Comfortable with independent decision-making, patient education, and EMR documentation
Thrives in fast-paced, travel-intensive roles and values teamwork and continuous learning
Day-to-day activities
Review daily caseload and route-plan for efficient home visits
Perform evaluations and one-on-one therapy sessions (strength, balance, mobility)
Educate patients and caregivers during home visits
Complete EMR charting between visits and coordinate with nurses and OTs to adjust plans based on real-time feedback
Team & collaboration
Report to the Clinical Manager of Home Health Services, with dotted-line collaboration to the regional therapy leader
Work alongside a multidisciplinary team of PTs, OTs, nurses, social workers, and support staff
Regular touchpoints with peers and field mentors for case discussion and support
